Job Summary: 


 


The Informatics Pharmacist (IT Pharmacist) is an expert in human factors, patient safety, and the use of technology to optimize care delivery processes and effectively communicate patient care activity. The IT Pharmacist specializes in the medication process, from ordering to administration, and is responsible for effective process design, development, and implementation of the Electronic Medical Record, Clinical Decision Support, automated dispensing cabinets, smart pump library, and other pharmacy technology systems. As well, this role supports pharmacists, physicians, nurses, and other clinicians as the resource for requests related to technology and EMR. The IT Pharmacist demonstrates independent decision making as appropriate in evaluation of requests for system improvements and determines feasibility and advisability of system changes. The IT Pharmacist works closely with clinicians, departmental leadership, Quality, Patient Safety, Accreditation and Licensure Departments and Information Systems staff to clarify needs, prioritize requests, design workflow to fit with electronic systems, and execute change and facilitate adaptation to the Electronic Medical Record. Instructs nursing staff on administration, and documentation of medications. Interprets physician prescriptions and medication orders.  Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department personnel.  Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties.  Performs other duties as assigned.


 


Duties Include:



  • Performs other duties as assigned by the Director.



  • As patient care and/or Hospital need dictates, both the assigned shift and work station may be changed by the department.

  • Participates in and promotes the mission and philosophy of the hospital.



  • The Informatics Pharmacist shall be responsible for maintaining, testing and updating Meditech dictionaries and functionality related to the pharmacy module, ePOM, eMAR CPOE,and other Meditech modules that interact with medication ordering, dispensing and/or administration.

  • The Informatics Pharmacist will be the primary pharmacy resource for the implementation and training of pharmacists, physicians and other healthcare providers in utilization of CPOE, ePOM and eMAR, ADCs, and other technology support systems.

  • The Informatics Pharmacist will interact closely with both corporate and facility personnel in the evaluation and design of new software functionality. 

  • Oversees implementation and support of various information technology systems concurrently.

  • Development and maintenance of overall principles for safe design and use of order sets, with a focus on medications.

  • Aggregates, prepares and presents routine Pharmacy data to team members, administration and regulatory agencies as necessary.

  • Responsible as thought leader for maximizing technology design to achieve the best clinical outcomes. Participates in the organizational design, implementation and evaluation of Clinical Decision Support.

  • Content expert on the literature related to the use of the Electronic Medical Record and Clinical Decision support to meet organizational quality and patient safety goals.

  • Partners with clinicians to identify and facilitate advanced clinical decision support implementation within the electronic medical record in order to meet clinical goals.

  • Participates in quality projects and on assigned committees to provide expertise on using the clinical information system to achieve goals.

  • Responsible for order set management:


    • Development and maintenance of standard order set clinical content

    • Development and maintenance of overall principles for safe design and use of order sets, with a focus on medications

    • Identified leaders as resource for requests for changes, additional orders and order sets

    • Shepherds clinical authors through the approval process outlines at hospital’s Informatics committees.

    • Collaborates with specialty authors to develop order sets based on order set standards, patient safety principles and evidence-based practice guidelines.


  • Provides orientation, training and continuous support to all staff as it pertains to the clinical system.

  • Presents information in logical, professional and appropriate manner. Presentation tools are current, applicable and clear.

  • Designs and develops system documentation tools. Assists in the performance of system testing with updates/upgrades in coordination with Corporate and facility testing.

  • Documents all changes and Dictionary builds with specific instructions for replication. Disseminates information regarding changes to the system to the appropriate end-users.

  • Identifies issues, investigates system issues to complete resolution and reports issues to the appropriate source with adequate follow up.

  • Proactively monitors the medication ordering process for errors and system malfunctions. Upon identification of system issues, is able to investigate the issue to resolution to the point of cause and then report the cause to the appropriate source.

  • Effectively educates physicians and other healthcare providers on ePOM and CPOE use and evaluates competency accurately prior to solo use.

  • Perform medication order review, verification, and dispensation

  • Assist in reviewing and improving medication management processes for regulatory compliance and quality improvement

  • Guide implementation of clinical initiatives such as antimicrobial and opioid stewardship, Core Measures, National Patient Safety Goals, reduced readmissions, etc. and serve on relevant committees

  • Assist with adverse event and medication error monitoring and medication safety measures

  • Provide education via training, newsletters, and communications to pharmacy staff and other disciplines

  • Responsible for updating and maintain departmental Intranet web page

  • Assist DOP in ensuring professional, productive, and efficient practices and as needed assist DOP in performing personnel evaluations

  • On 24 hour call as needed.

  • Precept students and/or residents

  • Assume the essential responsibilities and perform the duties of the DOP in the absence of the DOP/Clinical Manager unavailable.

Required Qualifications :

Requirements:




  • Either an informatics degree or informatics residency or experience equivalent to degree or residency plus the pharmacy degree



  • Graduated from an accredited School of Pharmacy.

  • Current Pharmacist certification or licensure in the State of Texas.

  • Hospital experience required.

  • Meditech experience preferred.

  • Relocation and call required.


  • Preferred- Informatics Pharmacy experience in acute care
