JOB DESCRIPTION for the case manager
The Case Manager is the Team Leader of a unit-based Case Management Team. Assumes overall responsibility for coordinating and managing patient-centered care activities for a designated cluster of patients during hospitalization to be accomplished with a multidisciplinary team. Initiates and directs Case Management of patients from pre-admission through discharge for appropriate movement through the continuum of care while providing quality care to meet specific patient needs. Monitors and coordinates a comprehensive program of admission reviews, continued stays and discharge plans of the patient. Evaluates, develops and implements discharge plans. Performs work process, which includes securing authorization/certification for acute care reimbursement coverage for patients. Monitors cases, develops, and implements plans for appropriate documentation and length of stay compliance.
JOB REQUIREMENTS for the case manager
Graduate of a school of Nursing, Bachelors in Nursing or related field preferred.
Two – three years of full-time clinical practice, with at least five years of nursing practice.
Experience with Case Management, discharge planning, Utilization Review, patient education, and home health preferred.
Current RN license to practice in the Commonwealth of Virginia or compact state.
Current AHA Healthcare Provider Certification required.
